The above merely summarizes the benefits of our outstanding Care Choices HMO program.

Our Dental Program covers 100% of diagnostic and preventative; 75% of restorative; 50% prosthetics; and the maximum co-pay for orthodontics is $1,995.00. Office visits do not require a co-payment.

Our Educational Assistance Program is designed to encourage hourly employees to increase their value within our Companythrough enrollment incareer oriented courses offered during non-working hours by accredited schools, colleges, and universities. Full-time employees with at least six (6) months service may participate in this program. The employee will be refunded after satisfactorily completing a course. Reimbursements are limited to the cost of tuition only.
